Letters of Suitability for ADW Clergy

• Testimonial for ADW Priests
  This form is to be used by Archdiocese of Washington priests who wish to exercise priestly ministry in another diocese. You may request this form by calling the Clergy Office, 301-853-4577.

Extern Priest Forms

• Testimonial – Visiting Priests or Students
  This form is to be used by those priests visiting to perform weddings, baptism, etc., without having an assignment or appointment given to them by the Archdiocese of Washington or for a student.
• Testimonial – Extern Priests with ADW Assignments
  This form is to be used by those priests who have an assignment or appointment given to them by the Archdiocese of Washington.
• Request for Faculties
  This form is to be used by all extern priests requesting faculties from the Archdiocese of Washington.
• Extern Priest Application
  This application is to be used by all priests requesting faculties in the Archdiocese of Washington.
• Child Protection Policy Booklet
  This booklet is to printed and read by all priests requesting faculties by the Archdiocese of Washington. Page 55 (Appendix E) must signed and returned to the Clergy Office.
• Testimonial – Visiting Deacon
  This form is to be used by those deacons visiting without having an assignment or appointment given to them by the Archdiocese of Washington or for a student.
